The tire came loose from a private sanitation truck.

A driver was killed in a freak accident on the Gowanus Expressway this morning when a tire flew off another vehicle and smashed into his car's windshield.
The driver, a civilian member of the NYPD, was heading east in the HOV lane near exit 3 when a tire somehow came loose from a private sanitation truck traveling west. According to WABC 7, "The tire struck and shattered the windshield of the Toyota RAV 4m, which then crashed into the divider" at approximately 6:30 a.m.
The driver, who was apparently a maintenance worker at 1 Police Plaza, suffered head trauma and was taken to NYU Langone, where he was pronounced dead. The driver of the sanitation truck stayed on the scene and has not been charged.
